Blepharoplasty

There is a significant technical difference between upper and lower eyelid correction. Both procedures are usually performed under local anesthesia.

Upper Eyelid Correction

To correct the fold of skin resting on the eyelashes, a crescent-shaped strip of skin is removed. This leaves a thin scar in the natural crease of the eyelid, which typically becomes completely invisible over time. If there's any bulging fat in the inner corner of the eye, it is removed at the same time. In many cases, the drooping of the tail of the eyebrow can also be corrected through the same incision (see also brow lift). Any hollowing in the upper eye socket can be filled using the patient’s own fat (see also lipofilling).

Lower Eyelid Correction

Unlike the upper eyelids, skin laxity is not always present in the lower eyelids. The “bags,” or bulging of fat beneath the eye, can almost always be corrected from the inside of the eyelid (this is a “transconjunctival” eyelid correction). If a dark groove under the eye remains, it can be filled with the patient’s own fat tissue (see also lipofilling). If the lower eyelid skin is loose or wrinkled, it can be smoothed by doing a chemical peel (see also chemical peel). If the skin is too stretched for a peel, a strip of skin may be removed via an incision just under the eyelashes. This scar also typically becomes invisible over time.

Recovery after eyelid surgery is usually quick and painless. In the first few days, the eyes may swell considerably, but this gradually decreases from the third day onward. Bruising can occur under the eyes. By the time the stitches are removed on day six, about two-thirds of the swelling has usually resolved. Bruising typically disappears within 2 to 3 weeks but can easily be concealed with makeup.

Heavy activities and bending over should be avoided during the first week. Aftercare includes applying an eye gel in the evening to protect the eyes from dryness. Any bothersome crusts may be gently removed with a moist cotton swab.

Almost everyone experiences dry eyes for 4 to 6 weeks after the procedure. This can be relieved by using hydrating eye drops during the day and applying the prescribed eye gel at night.

Bleeding is rare, but if one or both eyes suddenly swell and possibly start to bleed shortly after the procedure, always contact us immediately.

After lower eyelid correction, the eyelid margin may occasionally droop slightly (“ectropion”). This is usually temporary and can often be resolved with targeted massage. In rare cases, however, an additional correction may be needed. Your surgeon may then perform a “canthopexy,” a procedure that ensures the eyelid fits against the eyeball again.

In the early stages, you may notice that your vision is slightly less sharp. This is because the cornea, the part of the eye you see through, can temporarily swell. This may make activities like reading or watching TV more difficult, so it is advisable to limit them in the beginning. Your vision will recover on its own.
